How does it work?

Step 1. Sign up to our affiliate program. Click this link to fill out our affiliate registration form.

Step 2. Once we receive your application we will review it within 5 business days. If you meet our affiliate standards, you will receive an email notification.

Step 3. With our email notification you will receive a special link to download a W-9 form. Fill it out, sign and fax to 360-859-2410.

Once you become our affiliate we will provide you with our persuasive banner ads, articles and other marketing materials for you to effectively present our program to your contacts on your website.

Once someone visits your website and clicks on our banner, he or she will be forwarded to our website being identified as your referral.

When your referral purchases a Heart Wizard subscription, you earn your commission.
